Get paid back for covered care
If you paid out of your own pocket for things your plan covers — like healthcare services or Wellness Bucks purchases — you can send a reimbursement request to ask us to pay you back.
Once we get your request, we’ll pay you back for the amount your plan covers. We usually process requests within 30 days, but it may take up to 60 days.
Online
This is the quickest and easiest way to get paid back:
- Log in to the Devoted member portal and choose “Get reimbursed” — or text PAYMEBACK to 866-85 to get a direct link to the right page
- Enter your information and upload your receipt (and your detailed bill or invoice, for healthcare, dental, and vision purchases)
If you have a white and silver dental or dental/eyewear Mastercard, don't use this form. Log into your card’s account and make a payment request instead.
There are just a few cases where the process is different. Do NOT use this form for:
- Prescription drugs (Part D/pharmacy): Print and mail our Prescription Drug Reimbursement Form instead.
- Dental care (if the back of your ID card says Liberty Dental or Delta Dental): Print and mail the Liberty Dental Reimbursement Form or Delta Dental Reimbursement Form instead.

Don’t have a printer? Text us at 866-85 or call us at 1-800-338-6833 (TTY 711). We can mail you a copy.
The form is an easy way to make sure you’re sending us all the right information. But if you prefer not to use it, you can send us the same information on a separate sheet of paper instead.
