Welcome to Our Gold Care Program

Our care team will get to know you, learning what’s important to you in your healthcare and your life. Then, we’ll offer the support you need to help you meet your goals.

What can I expect from the program?

  • Regular visits with your care team over video and phone — we’ll decide together how often to meet based on your needs
  • Each visit takes 30 to 60 minutes
  • At these visits, your team makes sure that you and your caregivers have the services and resources you need to support your health

Who will be involved in my care?

Your Gold Care team includes your: 

  • Outreach coordinator to help schedule visits and check in with you
  • Community health worker to help you and your caregivers move through the healthcare system, use health plan benefits, and access community supports and services
  • Nurse case manager to provide support with your medical care, help you understand your health conditions, and make hospital stays as smooth as possible
  • Nurse practitioner or physician assistant to provide medical care and check that your overall care matches your needs
  • Social work case manager to make sure you and your caregivers have strong social support 
  • Doctor to advise and support you and your care team 

We’ll also send regular updates to your primary care provider (PCP) so everyone’s on the same page.

How can I get ready for my first visit?

  • Make a list of the names and phone numbers of any doctors you see regularly
  • Make a list of any upcoming healthcare appointments 
  • Make a list of your medications
  • Write down questions you want to ask 
  • Set up in a quiet, comfortable place where you can talk freely about your health 
  • Decide if you want anyone to join you for the call — like a family member, friend, or someone else who helps you with your health

How long will I be working with the clinic?

There’s no set timeframe! You’ll graduate from our program when you feel confident in your healthcare journey and you no longer need support from our team.
