Quality of care initiatives
We're committed to making sure our members get the best care possible. So we created programs to help our members with their specific health needs. Here’s how we did in 2023.
Chronic disease management
Diabetes
In this program, we help members with uncontrolled diabetes better manage their symptoms so they feel healthier. We work with both the member and their primary care provider (PCP), focusing on custom goals and important screenings. In 2023, the program saw excellent results:
- Members who graduated from the program lowered their HbA1c by an average of 2.3%
- More than 81% of our members received an eye disease screening
High blood pressure (Hypertension)
In 2023, our Hypertension successfully helped members lower their high blood pressure. Approximately 64% of members who enrolled in the program graduated with their blood pressure under control.
Medication adherence
Medication adherence means taking your medications just as your doctor tells you to. This is a key step in controlling chronic conditions like high blood pressure, diabetes, and high cholesterol.
Staying on track with medications is hard. Our Clinical Team and Devoted Health Guides provide personalized support to our members and their care teams (including providers and pharmacies) to help improve medication adherence. In 2023, 92% of our members with high blood pressure, high cholesterol, or diabetes successfully took their medications as directed.
Transitions of care
Going home from the hospital can be a stressful and difficult time. Our transitions of care program continued in 2023, focusing on helping members who leave a hospital or skilled nursing facility. Our team works with members to coordinate their care and help with clinical and social needs, preventing hospital readmissions and other barriers to a successful at-home recovery.
