Appointment of Representative

You can have a friend, family member, lawyer, or anyone else you trust help you with an appeal or complaint. But first you need to appoint them as your representative — that gives them the legal right to act for you.

You just need to complete the Appointment of Representative form and send it back to us. (If you already have a durable power of attorney, you're all set — you don’t need this form.)
